Registration may be completed by contacting us to receive the forms for the specific program you

We will need to have the following information for you to be placed on our active registered list.
These forms must be filled out and returned to our office prior to your acceptance in the program.

1. Personal Information Form 2009 (.doc)
2. Release Liability for 2009 (.doc)
3. Copy of the photo page of your passport.
4. Receipt of your deposit either by personal check, money order or bank wire.
5. Call us to receive information for bank wires of your deposit and payment. This has proven to be the easiest method of making your payments. It is also the least expensive.
